Strength and Conditioning Programs: How to Make Change Easier

Yesterday was a busy (but fun) day at Cressey Performance, and when I got home around 7pm, I was beat. Luckily, it doesn’t take much energy to check emails, so that’s what I did.  This one made my night: Hey Eric, Just wanted to thank you for helping me out this summer. I've weighed in at 197 the last few days, a 19 pound increase in about 3 months. My fastball has gone up 7-8 mph and I still feel like I haven't thrown the ball near my best yet. Because of the work I put in this summer I now have a legitimate shot to pitch a lot this year after not seeing an inning and getting redshirted last season. Thanks again, John Pretty cool, huh?  These are the kind of emails that make the long days all worthwhile and remind me why I have the coolest job in the world.  It gets better, though – as there is a lot to be learned from this specific story. John – a college pitcher coming off two surgeries in two years on his throwing shoulder, plus a few hamstrings pulls – drove seven hours for his one-time consultation/evaluation at Cressey Performance back in May and then took a program home with him.  Then, he drove back to CP at the start of his June and July programs to learn the exercises and check in with us to make sure everything was progressing nicely.  That’s some serious dedication (and gas money!).

Just as significant, though, was his ability to embrace change, as our programs were a huge deviation from his previous experiences.  His original email to us included this line: “I run 6 days a week, one of my goals between the end of this season and the beginning of next one is to run 1,000 miles.”  He didn’t do a single “run” over 50 yards in the entire three month program with us.  He also did far more (and longer) long toss in his throwing program than he had previously.  So, you could say that he not only embraced a change, but thrived with it. Change is tough, though.  Lots of people read my blogs, hear me speak at seminars, and interact with me on short-term observational visits to Cressey Performance – but only a small percentage of them actually put things into action.  Loads of people acquire knowledge, but never act on it. However, interestingly, when a new client starts up at CP, they stand a much better chance of succeeding with change.  Starting (and staying consistent with) a strength and conditioning program is a big undertaking; in fact, for many, it’s as significant as taking on a new job, opening a new business, or learning to play a new sport or instrument.  And, when that program is a complete deviation from what you’re expecting, it’s even tougher. Why, then, do some people succeed with change more than others?  I think it has to do with a lot of factors, but these five stand out the most to me: 1. They get those around them involved – John’s dad came along for the ride for his first day at CP – and this is often the case for the parents of our high school athletes.  While you don’t want overbearing parents, you do want a support system that’s aware of new goals and can be there to help keep one accountable in the quest for change.

2. They find good training partners and a quality training environment – I had a quick video blog about this yesterday, but I’m convinced that training partners and environment are just as important as an effective program.  There are always people to pick you up when you’re dragging, and the energy is contagious.  It makes change fun while making it seem like it is actually a “norm,” as training partners are constantly reaffirming what you’re doing and providing encouragement and feedback. 3. They don’t get overwhelmed by changing everything – Sometimes, the easiest way to create massive change is to take baby steps and break the overhaul into smaller components.  As I wrote recently, small hinges swing big doors.  This has never been my “cup of tea,” but there have been times when we’ve had to slowly change around a program for a client that was accustomed to a completely different school of thought.  “One of mine and one of yours” can work for the initial period and help you to gain an individual’s trust before a more thorough transition. 4. They incorporate this change into an existing schema – This is one I originally read in the book Switch: How to Change Things When Change is Hard, by Chip and Dan Heath.

To illustrate things, I’ll call upon my own personal experience.  Back in 2006 or so, I didn’t think that there was any possible way that semi-private training could work. How could you have clients of all different ages, experience levels, and goals training at the same time without having chaos?  My buddy, Alwyn Cosgrove (who, at the time, had just beaten stage 4 cancer for the second time), had some great advice: Physical therapy is done in group settings. Cardiac and pulmonary rehab are done in group settings.  I did pulmonary rehab post-chemo.  Seventeen of us in the group and one nurse. That's called semi-private! Chemotherapy is done in a semi-private setting for most cancers, too. My first time through there were ten of us in a room with two nurses. Actually, when I was in the hospital getting chemo it was still semi-private. I had one nurse who covered six rooms. Now I'm even more convinced. If life saving (and potentially deadly chemotherapy) is done in a small group setting, you're really stretching to tell me that an exercise program has to be one-on-one. We now do almost exclusively semi-private training, and it’s amazing.  Middle school athletes get to watch how the high school guys train.  The pro guys get to mentor the high school guys.  The adult clients get to know athletes they see on TV on a personal level.  Experienced clients introduce themselves to new clients when they start training.  Just the other day, one of our local families had two of out-of-town athletes (Colorado and Virginia) over for dinner on Saturday night, and then brought them to church with them on Sunday morning.  There is insane camaraderie among folks from all different walks of life.

None of it would have been possible if I hadn’t been able to wrap my head around the idea of semi-private training – and it would have been tough to get to that point if Alwyn hadn’t put the concept into my existing schemas (physical therapy, cardiac/pulmonary rehab/chemotherapy) for me. 5. They spend money – Taking a leap of faith and increasing the stakes can sometimes motivate people to make change happen.  Whether it’s a payment for training, or just a bet with friends about exercise consistency or some training goal, separating people from their money always seems to magically increase adherence.  People don’t like getting ripped off – and it’s even worse when you rip yourself off because there is nobody else to blame except yourself! In a recent example, Pat Rigsby, Mike Robertson, and I outline many assessment, training, and business strategies that one can effectively employ in a fitness business in The Fitness Business Blueprint.  One of our primary goals in making it the way that we did was to make sure that we made it easier for buyers to apply the changes we recommended; we discussed how to incorporate our ideas seamlessly in their current business strategy.  Still, none of these tactics will work is someone isn’t willing to change – and that means putting in some leg work to both set the stage for change and then follow through on it.

Understanding and Managing Fatigue

Perhaps it’s coincidence, or perhaps the scientific community is finally catching on, but recently, there have been several studies looking at the role of short- and long-term recovery in preventing and rehabilitating injuries.

Here’s a research study that demonstrates relationships among a variety of scheduling and recovery factors and injury rates. The part I found most interesting was that researchers observed that sleeping fewer than six hours the night prior to a competition led to a significant increase in fatigue related injuries.

Additionally, researchers at Stanford recently demonstrated the profoundly positive effect that “sleep extension” has on a variety of performance variables in high-level basketball athletes.

These results, in themselves, aren’t particularly surprising: fatigue impacts performance – whether that’s on the field, or in the rehabilitation realm. Anyone who has ever trained an athlete on a Saturday morning after he’s had a late Friday night, or rehabbed a roofer after he’s completed a 10-hour-workday, will tell you that there are certainly less-than-optimal times to get the work in.

What research like this doesn’t tell us, though, is that not all fatigue is created equal – and I suspect that this is one area where strength and conditioning specialists can “return the favor” to rehabilitation specialists for all that we’ve learned from them over the years. Very simply, the very best strength and conditioning coaches I know are the ones who are masters of managing competing demands, including strength training, mobility drills, soft tissue work, movement training, metabolic conditioning, and sport-specific training. In order to effectively manage all these factors, it’s imperative to understand the different stages of fatigue. On the rehabilitation side of things, every injured athlete likely has some element of fatigue that not only impacted his/her injury mechanism, but will impact the response to a given rehabilitation program.

Over-what? Over-everything!

In their classic review, The Unknown Mechanism of the Overtraining Syndrome, Armstrong and VanHeest discussed the importance of differentiating among overload, over-reaching, overtraining, and the overtraining syndrome (OTS). They defined the terms as follows:

  • Overload – “a planned, systematic, progressive increase in training stimuli that is required for improvements in strength, power, and endurance”
  • Over-reaching – “training that involves a brief period of overload, with inadequate recovery, that exceeds the athlete’s adaptive capacity. This process involves a temporary performance decrement lasting from several days to several weeks.”
  • Overtraining – training that “exceeds over-reaching and results in frank physiological maladaptation(s) and chronically reduced exercise performance. It proceeds from imbalances between training and recovery, exercise and exercise capacity, stress and stress tolerance; training exceeds recovery, exercise exceeds one’s capacity, and stressors exceed one’s stress tolerance.”
  • Overtraining Syndrome (OTS) – “a set of persistent physical and psychological symptoms that occur subsequent to prolonged application of heavy training loads. The critical diagnostic factor is a chronic decrease in performance, not simply the existence of SAS [signs and symptoms].”

Overload is inherent to a successful training process, and over-reaching is actually quite valuable when used appropriately. For instance, in our training programs at Cressey Performance, we generally fluctuate training stress in four-week programs as high (1), medium (2), very high (3), low (4), where the deloading in week 4 allows for adaptation from the fatigue imposed during week 3.

However, over-reaching is far from overtraining – a term that is thrown around far too often among even the most qualified individuals in the world of health and human performance. Over-reaching may be attained in as little as 7-10 days, and remedied in a matter of days or weeks with adequate deloading. Conversely, the process of overtraining must take place for months for the outcome, OTS, to be apparent. Recovery from OTS requires at least several weeks – and more often several months; in other words, you really have to go out of your way to get to overtraining syndrome.

Since high level performance – and even just normal physical health – is a priority, it is imperative that coaches, parents, and athletes recognize the signs and symptoms of over-reaching and overtraining syndrome – and the differences between the two. According to Armstrong and VanHeest, the signs and symptoms of OTS may include:

  • Decreased physical performance
  • General fatigue, malaise, loss of vigor
  • Insomnia
  • Change in appetite
  • Irritability, restlessness, excitability, anxiety
  • Loss of body weight
  • Loss of motivation
  • Lack of mental concentration
  • Feelings of depression

What All These “Overs” Mean to You

Many of these signs and symptoms are shared between over-reaching and OTS, so how do we know the difference? How do we know when to hold back for a day or two (for overload recovery), 7-21 days (over-reaching), or even months (overtraining syndrome)?

Unfortunately, as much as I would like to be able to offer you the magic answer, I can’t do so. The scientific community has yet to agree on a single, highly sensitive diagnostic test to differentiate among the three. In fact, the only diagnostic tests that are universally accurate are those of physical performance; if performance drops off, there must be some degree of accumulated fatigue.

Other measures – such as heart rate, bloodwork, metabolic rate, substrate metabolism, and a host more – are subject to so many factors that they are hardly reliable tests of one’s training status.

As an example, research from Fry et al. had subjects perform ten sets of one repetition on machine squats at 100% of their one-rep maximum for 14 days straight. That’s an absurd volume of high-intensity resistance training, especially in a trained population. You know what, though? The only thing that dropped off was performance; hormone status (as measured by bloodwork) really didn’t change much at all.

Conversely, crush an endurance athlete with volume, and this same bloodwork will look terrible. The take-home point is that it’s a lot harder to “overtrain” on intensity than volume. And that’s where the problem exists when you’re dealing with athletes: just about every sport out there is a blend of volume and intensity. We don’t just train or rehabilitate shotputters or Ironman competitors; we get athletes from soccer, basketball, baseball, hockey, tennis, and a host of other sports.

So, what is a coach or rehabilitation specialist to do when trying to determine just how much fatigue is present, and what the best course of action is to guarantee an optimal return-to-play as quickly as possible?

In two words: ask questions.

In my opinion, the absolute most important step is to establish communication with athletes and – in this case – patients. Ask about training practices before an injury, sleep patterns, dietary factors, family life, concurrent illness/injury, changes in body weight, and appetite.

These may seem like obvious questions to ask, but we live in a one-size-fits-all world of pre-made templates and rigid systems – and people can fall through the cracks all the time. My experience has been that those most commonly “thrown under the bus” in this regard are the most dedicated athletes forced to train or rehabilitate in a “general health” world. As an example, we had an adult athlete client request a Vitamin D test from a primary care physician last year, and he was turned down because he wasn’t “a post-menopausal female.” As it turned out, he was severely clinically deficient, and normalizing his Vitamin D was a big game-changer for him.

Simply asking the right questions will always help the cause when it comes to determining just how “systemic” what you’re dealing with really is. And, in the process, it gives you an opportunity to show a client or patient how much you care before they even care how much you know.

Youth Strength and Conditioning Programs: “He’s a Big, Strong Kid.”

Recently, while discussing one particular athlete we encountered at Cressey Sports Performance, my staff members and I got on the topic of how it's more of a challenge to train bigger (taller and heavier) young athletes than it is to work with smaller guys.  Interestingly, the challenges come less from the actual physical issues they present and more from the social expectations that surround their size. Here are seven reasons why I cringe when I hear parents say "he's a big, strong kid" when describing their children on the phone.

1. Bigger kids are often forced into sports and positions that may impede their long-term development - When you're the heavy kid, you're automatically pushed toward football and put on the line.  If you're playing baseball, it's first base or catcher.  If it's basketball, you're the power forward.  You get the picture - and similar "pushes" are made on tall kids to play basketball or volleyball.  The problem is that in most cases, these sport and positional "predispositions" put bigger kids in situations where they don't develop in a broad sense because there simply isn't enough variety.

2. Bigger kids usually start weight-training on their own - This point relates closely to point #1.  Unfortunately, when you're already labeled as the next star offensive lineman or power forward and you can already push your buddies around, chances are that you learned to lift with Dad in the basement, from a misinformed football coach, or be screwing around with your buddies.  I would much rather have a completely untrained 16-year-old start up with me than be presented with a 16-year-old with years of poor strength and conditioning programs and coaching under his belt.  This is true regardless of body type, but especially problematic in bigger kids for reasons I outline below.

3. "Strong" has different meanings - Sports require a combination of absolute and relative strength.  Strength is also highly specific to the range of motion (ROM) in which one trains. There is also a difference between concentric and eccentric strength.

What do most big young athletes do when left on their own?  Focus heavily on absolute strength (train what they're good at) through small ROMs (rather than fight their bodies) with concentric-heavy workloads (because pushing a blocking/tackling sled is sexier than a properly executed lunge).

I can count on one hand the number of teenage athletes who were called "big and strong" who have actually showed up on their first day and demonstrated any appreciable level of strength in any context - let alone usable strength that will help them in athletic endeavors.  Usually, we wind up seeing a sloppy 135-pound bench press with the elbows flared, legs kicking, bar bouncing off the chest...in a kid who can't do a push-up.

And this is where the problem arises: kids who have always been told they were strong don't like coming to the realization that they really aren't strong.  We don't have to directly tell them, either; taking them through basic strength exercises with proper form will reveal a lot.  And, there is typically an example of a smaller athlete like this kicking around not too far away.

The kids who check their egos at the door will thrive.  A lot might never come back until they're injured from poor body control or riding the pine because it turns out that their "strengths" really weren't that strong.

4. Bones grow faster than muscles and tendons - In young athletes who haven't gone through the adolescent growth spurt, you often don't have to do any additional static stretching, as a dynamic warm-up and strength training program through a full ROM can cover all their mobility needs.  Unfortunately, when kids grow quickly, the bones lengthen much faster than the muscles and tendons do, so we run into situations where bigger kids have truly short (not just stiff) tissues.  Effectively, this adds one more competing demand for their time and attention - and it's the worst kind to add, as most kids hate to stretch.

5. Being bigger changes one's stabilization strategy -  As I described in great detail in The Truth About Unstable Surface Training, the taller one is, the further the center of gravity is away from the base of support.  As such, taller kids are inherently more unstable than shorter kids - although this can be partially remedied by gaining muscle mass in the lower body to lower the COG and learning to "play lower" in appropriate situations.

Not surprisingly, though, being heavier - particularly with respect to having a belly - can dramatically change one's stability as well.  Carrying belly fat shifts the center of gravity forward - which is why individuals with this "keg" instead of a six-pack appear more lordotic (excessively arched at the lower back).  Compensations for this occur all along the kinetic chain, but the two things I'd highlight the most are:

a. An increased need for anterior core strength - As evidenced by the high incidence of spondylolysis (lumbar spine fractures) and how badly most kids perform on basic prone bridging and rollout challenges, the inability to resist lumbar hyperextension (and excessive rotation) is a serious problem.  The bigger the belly, the more extended the lumbar spine will be.  Just ask any pregnant woman how her back feels during the last trimester.

b. Substitution of lumbar (hyper)extension for hip extension - You'll see a lot of big-bellied kids who can't fully extend their hip and instead just arch their back to get to where they need to be.  This is a problem on multiple fronts.  First, the hip extensors are far stronger and more powerful than the lumbar extensors, so performance is severely impaired.  Second, there are huge injury implications both chronically (lumbar stress fractures, hip capsule irritation) and acutely (strained rectus femoris or hamstrings).

Simply dropping some body fat and improving anterior core strength is a huge game-changer for many overweight athletes.  It's not always the answer they want to hear.

6. Bigger kids usually have less work capacity - I've never been a guy who jumped on the work capacity bandwagon, as I feel that it's very activity-specific.  However, it doesn't take a rocket scientist to observe that the more body fat one carries, the more work he'll do oxygenating useless tissue, and the less oxygen he'll get to working muscles.  More importantly, though, try doing your next training session with a 60-pound weight vest on and see what it does to your work capacity.

The lower the work capacity, the less quality work one can accomplish in a strength and conditioning program.  Gains simply don't come as quickly on the strength and fitness side of things - even if body fat is pouring off heavier athletes.  In other words, they've actually sacrificed one window of adaptation (athletic development) in order to make another one (fat loss) larger.

7. I speculate that bigger athletes have an increased prevalence of "subclinical" musculoskeletal pathologies/deviations from normalcy - I've written in the past about how many athletes are just waiting to reach threshold because their MRIs and x-rays look terrible - even if they are completely asymptomatic.  You can see this just about anywhere in the body; most basketball players are just waiting for patellar tendinosis to kick in, and many football lineman are teetering on the brink of a lumbar stress fracture or spondylolisthesis (or both).

The heavier one is - especially in the presence of insufficient relative strength, as noted above - the more pounding one will place on the passive restraints such as the meniscus, intervertebral discs, and labrum.  A bigger belly and the resulting lordosis will drive more anterior pelvic tilt, femoral/tibial internal rotation, and pronation.  How would you like to be the plantar fascia or Achilles tendon in this situation?

Tall athletes tend to slouch more because they have to look down at all their peers.  Get more kyphotic, add some scapular dyskinesis, and see what happens to the rotator cuff, labrum, and biceps tendon over time.

There are countless examples along these lines.  And, to make matters worse, obese individuals are more likely to have inaccurate diagnostic imaging.  In an interview I did with radiologist Dr. Jason Hodges, he commented:

By far, the biggest limitation [to diagnostic imaging] is obesity. All of the imaging modalities are limited by it, mostly for technical reasons. An ultrasound beam can only penetrate so far into the soft tissues. X-rays and CT scans are degraded by scattered radiation, which leads to a higher radiation dose and grainy images. Also, the time it takes to do the study increases, which gives a higher incidence of motion blur.

I want to be very clear; I love dealing with bigger kids just like I do all my other athletes.  We don't lock them in a closet with celery sticks and an exercise bike; we work them hard, but make training fun and support them fully in their quest to fulfill their athletic potential.  Having been an overweight teenage athlete myself, I know that weight management in young athletes is a hugely sensitive subject that must be approached with extreme care.

I also know, however, that in my overweight years, I would have much rather been worked hard like the other athletes and given the opportunity to choose my sport and position of interest rather than pigeonholed into one specific avenue because of my build.  That's where the "big, strong kid" label really concerns me and makes me want to plan out my strategy - both in terms of the physiological and social approach to training - very carefully.

Superhero Workout Review: Small Hinges Swing Big Doors

I was reading a book Sunday afternoon, and this sentence really caught my attention: “Small hinges swing big doors.” Certainly, this is wildly applicable to just about every aspect of life, but particularly to strength and conditioning programs and nutritional approaches.

All too often, folks think that they need to overhaul what they’re doing because they’re stuck in a rut.  They switch from traditional sets and reps to high-intensity training, go on some fad diet, drop $200 at the supplement store on herbs they can’t pronounce, and buy a pair of “toning” shoes.   In short, instead of using the small hinge, they kick down the damn door.  The end results? 1. a thinner wallet (always) 2. continued poor results (almost always) 3. positive results (rarely), but with no idea which of the changes led to these outcomes

The longer I’m at it, the more I realize that long-term success in strength and conditioning programs is all about understanding how to change the hinges: finding the little things that make the big difference.  Maybe it’s a reduction in training volume or intensity to keep someone from burning out, or switching to a reverse lunge instead of a forward lunge to avoid knee pain. Case in point, John Romaniello and Matt McGorry recently sent me an advanced copy of their new Superhero Program, so we decided that we’d make it the staff lift at Cressey Performnace, as it looks really solid.  Plus, we’re super busy at CP right now, so it’s nice to be able to “outsource” our own training for the time being.  We aren’t overhauling our diets or supplementation regimens, nor are we introducing a ton of new exercises; in fact, most of the exercises in the program are ones we do on a regular basis at CP (although many will be novel to others). The program is, however, changing some of the hinges on our doors, particularly in the context of challenging set/rep/tempo protocols and novel fluctuation of training stress from phase-to-phase.  The only things that changed were, in fact, written on a piece of paper – but they got big doors in motion.

There was great energy in the facility today because the guys were excited to try something new.  And, there was more camaraderie among our staff because guys were coaching each other through things and shouting encouragement as we were all “feeling out” the new program. And, judging from the soreness that’s slowly setting in as I write this roughly 10 hours after the first training session of the program, it’s going to be a fun, challenging, and productive few months on a great program.  No overhaul needed – because small hinges swing big doors. The next time you find yourself looking to shuffle things up, remember that unless you’re a true beginner doing everything incorrectly, you usually don’t need to change a lot.  Rather, you pick and choose your modifications – or look to a resource like the Superhero program that has the important components in place, but perhaps in a light you hadn’t considered them before. Corrective Exercise: Sequencing the Law of Repetition Motion Sequence

When it comes to corrective exercise programs, everyone simply wants to know "what" is and isn't included - and rightfully so. Picking the right strength exercises and mobility drills - and contraindicating others - is absolutely crucial to making sure you get folks to where they want to be. However, very rarely will you hear anyone specifically discuss the "when" in these scenarios, and as I'll demonstrate in today's piece, it's likely just as crucial to get this aspect correct. To begin to illustrate my point, I'm going to reuse a quote from an article I wrote a few weeks ago, Correcting Bad Posture: Are Deadlifts Enough?, on the Law of Repetitive Motion : Consider the law of repetitive motion, where “I” is injury to the tissues, “N” is the number of repetitions, “F” is the force of each repetition as a percentage of maximal strength, “A” is the amplitude (range of motion) of each repetition, and “R” is rest.  To reduce injury to tissues (which negative postural adaptations can be considered), you have to work on each of the five factors in this equation.

You perform soft tissue work – whether it’s foam rolling or targeted manual therapy – on the excessively short or stiff tissues (I).  You reduce the number of repetitions (length of time in poor posture: R), and in certain cases, you may work to strengthen an injured tissue (reduce F).  You incorporate mobility drills (increase A) and avoid bad postures (increase R). What I failed to mention a few weeks ago, though, was that the sequencing of these corrective modalities must be perfect in order to optimize the training/corrective effect and avoid exacerbating symptoms.  Case in point, we recently had a client come to us as a last resort with chronic shoulder issues, as he was hoping to avoid surgery.  Physical therapy had made no difference for him (aside from shrinking his wallet with co-pays), and following that poor outcome, he'd had a similar result with soft tissue treatments twice a week for six weeks.  In a single four-week program, we had him back to playing golf pain free.  What was the difference?

In the first physical therapy experience, he'd been given a bunch of traditional rotator cuff and scapular stabilization exercises.  There had been absolutely no focus on soft tissue work or targeted mobility drills to get the ball rolling.  In other words, all he did was improve stability within the range of motion he already had.  In the equation above, all he really worked on was reducing the "F" by getting a bit stronger. In his soft tissue treatment experiences, he felt a bit better walking out of the office, but ran into a world of hurt when his provider encouraged him to "just do triceps pressdowns and lat pulldowns" for strength training.  In other words, this practitioner worked on reducing "I" and increasing "A," but totally missed the boat with respect  to enhancing strength (reducing "F") and increasing rest ("R") because of the inappropriate follow-up strength exercise prescription.  Doh!

What did we do differently to get him to where he needed to be?  For starters, he saw Dr. Nate Tiplady, a manual therapist at CP, twice a week for combination Graston Technique and Active Release treatments (reducing "I") at the start of his training sessions.  He followed that up with a specific manual stretching, positional breathing, and mobility exercise warm-up program (increase "A") that was designed uniquely for him.  Then, he performed strength training to establish stability (decrease "F") within the new ranges of motion (ROM) attained without reproducing his symptoms (decreasing "N" and increasing "R). The sequencing was key, as we couldn't have done some of the strength exercises we used if we hadn't first gotten the soft tissue work and improved his ROM.  He may have had valuable inclusions in his previous rehabilitation efforts, but he never had them at the same time, in the correct sequence. This thought process actually closely parallels a corrective exercise approach Charlie Weingroff put out there much more succinctly in his Rehab = Training, Training = Rehab DVD set: Get Long. Get Strong. Train Hard.

Keep in mind that there are loads of different ways that you can "get long."  You might use soft tissue work (Active Release, Graston Technique, Traditional Massage, etc.), positional breathing (Postural Respiration Institute), mobility drills (Assess and Correct), manual stretching, or any of a host of other approaches (Mulligan, DNS, Maitland, McKenzie, etc).  You use whatever you are comfortable using within your scope of practice.

When it's time to "get strong," you can do so via several schools of thought as well - but the important thing is that the strength exercises you choose don't provoke any symptoms.

It's interesting to note that this corrective exercise approach actually parallels what we do with our everyday strength and conditioning programs at Cressey Performance - and what I put forth in Show and Go: High Performance Training to Look, Feel, and Move Better.  We foam roll, do mobility warm-ups, and then get cracking on strength and stability within these "acutely" optimized ranges of motion to make them more permanent.

2 Key Strategies for Creating Effective Strength and Conditioning Programs

One of the questions I'm asked the most frequently is "How did you learn how to write strength and conditioning programs?" Unfortunately, while it's a tremendously common question posed to me, I haven't yet determined a quick and easy response that would work for everyone.  While this may make it seem like I haven't learned anything in this regard, the truth is that I get more and more effective and efficient with creating programs every single day.  What's my secret?  Well, I actually have two of them.

1. I NEVER reinvent the wheel. Our philosophies are constantly evolving, and I'm always working to integrate new concepts into our programming to improve outcomes for our clients.  These ideas may come from things I've read, seminars I've attended, other programs I've observed, or - most importantly - feedback our athletes have given us.  I absolutely, positively, NEVER overhaul a program, though.  Why? If you change everything, you learn nothing - because you can never appreciate what modification it was that worked (or didn't work). 2. I build on previous successes, rather than starting from scratch with every new client. I absolutely loved the book Made to Stick: Why Some Ideas Survive and Others Die, by Chip and Dan Heath.  I read this book back in early 2007, and I still refer back to it all the time.

One of the key points that the Heaths make in this book is that an idea will always be more readily accepted if it is incorporated into an individual's existing schema.  As an example, if I give you the letters TICDGFASOH and then ask you to list all the letters I included to me 20 minutes later without writing them down, most of you won't be able to accomplish the task correctly.

However, if I reordered those letters as CATDOGFISH, you'd accomplish the task easily.  You know the words DOG, CAT, and FISH - so it would fit into your existing schema.  This applies to strength and conditioning programs, too.

When I attend a seminar, whenever a new technique is introduced, I try to immediately apply it in my notes and in my brain to an existing client of mine.  How can that subtle modification make this individual's program better?

And, when I evaluate a client for the first time, I ask myself how this client is similar to a previous client of mine.  I'll look back to that old client's program to see what we used to get results - and then I'll tinker accordingly based in the new client's more specific individual needs.  I absolutely NEVER open up a blank Microsoft Excel template and write something from scratch, as it's always easiest to tinker with what's worked in the past.

What does this mean for the up-and-coming strength and conditioning program author?

Get out to as many seminars as possible.  Visit other coaches and observe their programs.  Read books and watch DVDs to learn about how others incorporate different strategies and strength exercises in their weight training programs.  Your goal is to expand your existing schema as much as possible and - in the beginning - create the strength and conditioning programs that will end up becoming the foundation for all future programs.  After the first few months, you are simply "tinkering," not overhauling.  Never reinvent the wheel, and always build on previous successes.

Youth Strength and Conditioning Programs: Systems, Not Just Sets and Reps

Back in November of 2010, a good buddy of mine who is a very accomplished college strength coach came up to Boston for a seminar we were hosting at Cressey Sports Performance.  The seminar was on a Sunday, but he actually flew up Friday night so that he could observe on Saturday while we trained our clients – which was a nice blend of high school, college, and professional athletes, plus our adult clientele.  All told, at the time, I’d say that high school athletes were 70% of our clientele.

That Tuesday morning, I woke up to this email from him:

“I just wanted to say thanks for everything.  I had a great time.  Your staff was outstanding and I really enjoyed watching you guys work on Saturday.  I realize you are managers, but certainly technicians as well.  Perfect form, I told Tony I saw two bad reps all weekend and someone was on the athlete before he had a chance to do another rep!!!   Thanks so much and come visit anytime, we would love to have you.”

This isn’t an email to toot our own horn; it’s to make a very valuable point.  If this coach had walked into every single private training facility and high school weight room in the country, in what percentage of cases do you think he would have come out with a favorable impression of the technique he witnessed in these strength and conditioning programs?  If I had to venture an extremely conservative guess, I’d say less than 10%.

Simply stated, both in the public and private sectors, some coaches are letting kids get away with murder with respect to technique, not warming up, poor load selection in weight training programs, and a host of other factors.

What happens, then, when the s**t hits the fan and a kid gets hurt?  I’ll tell you: certain exercises get “condemned” and strength and conditioning programs become more and more foo-foo; external loading is eliminated and kids wind up doing agility ladders and “speed training” for 60-90 minutes at a time in what can only be described as glorified babysitting.  Or, worse yet, weight rooms get closed altogether.  The door of opportunity gets slammed in the faces of a lot of kids who desperately need to get strong to stay healthy, improve performance, and build confidence.

That’s the reactive model, but what about a proactive model to prevent these issues in the first place?  Again, I’ll tell you: assess kids up-front.  Find out what is in their health history and evaluate how well they move.  Actually learn their names and backgrounds.  Then, program individually for them.  Coach intensely in their initial sessions and get things right from the start.  And, if an exercise doesn’t work for them, give them an alternative.

As an example, take the squat.  Some kids may not have sufficient ankle or hip mobility to squat deep in an Olympic style squat, so they’ll benefit more (and stay healthier) with box squat variations or single-leg exercises while you improve their mobility.  Others may even be too immobile (or possess structural issues like femoroacetabular impingement) to even box squat safely, so you give them more single-leg work and deadlift variations.  Regardless, you “coach ‘em up” well from the get-go – and they learn along the way.

In other words, the exercises aren’t the problem because exercises can be quickly and easily changed on the fly to match the athlete's level of abilities.  It’s the system in which they are placed that can be the stubborn, tough-to-change problem.

If you're struggling to get results with your youth strength and conditioning programs - or your business itself is struggling - be sure to look at your business model and overall systems before you start tinkering with the individual exercises.  Chances are that you need to rededicate yourself to relationship building and individualization more than you need to worry about sets and reps.

Pitching Injuries: It’s Not Just What You’re Doing; It’s What You’ve Already Done

A while back, this article on pitching injuries became the single-most popular piece in EricCressey.com history:

Your Arm hurts?  Thank Your Little League, Fall Ball, and AAU Coaches

In that feature, I made the following statement:

We can do all the strength training, mobility work, and soft tissue treatments in the world and it won’t matter if they’re overused – because I’m just not smart enough to have figured out how to go back in time and change history. Worried about whether they’re throwing curveballs, or if their mechanics are perfect?  It won’t matter if they’ve already accumulated too many innings.

While athletes might be playing with fire each time they throw, the pain presentation pattern is different.  You burn your hand, and you know instantly.  Pitching injuries take time to come about. Maybe you do microscopic damage to your ulnar collateral ligament each time you throw – and then come back and pitch again before it’s had time to fully regenerate.  Or, maybe you ignore the shoulder internal rotation deficit and scapular dyskinesis you’ve got and it gets worse and worse for years – until you’re finally on the surgeon’s table for a labral and/or rotator cuff repair.  These issues might be managed conservatively if painful during the teenage years (or go undetected if no pain is present) – but once a kid hits age 18 or 19, it seems to automatically become “socially acceptable” to do an elbow or shoulder surgery.

Sure enough, just yesterday, reader Paul Vajdic sent me this article from the Shreveport Times. The author interviews world-renowned orthopedic surgeon Dr. James Andrews about the crazy increase in the number of Tommy John surgeries he'd performed over the past decade.

A comment he made really jumped out at me, in light of my point from above:

""I had a kid come in, a 15-year-old from Boca Raton, (Fla.), who tore his ligament completely in two,' Andrews said. 'The interesting thing is when I X-rayed his elbow with good magnification, he has a little calcification right where the ligament attaches to the bone. We're seeing more of that now. He actually got hurt with a minor pull of the ligament when he was 10, 11, 12 years of age. That little calcification gets bigger and, initially, it won't look like anything but a sore elbow. As that matures, it becomes more prominent. It turns into an English pea-size bone piece and pulls part of the ligament off when they're young.'"

In other words, it takes repeated bouts of microtrauma over the course of many years to bring an athlete to threshold - even if they have little to no symptoms along the way.  Injury prevention starts at the youngest ages; otherwise, you're just playing from behind the 8-ball when you start training high school and college players.

In addition to walking away with the perspective that young kids need to be strictly managed with their pitch counts, I hope this makes you appreciate the value of strength and conditioning programs at young ages, too.  For more information, check out my post, The Truth About Strength Training for Kids.

Strength and Conditioning Programs: Trust vs. Self-Reliance

Back in graduate school, an opportunity arose to invest in an up-and-coming company.  At the time, I was swamped with student loans and really didn’t have the $5,000 “buy-in” to spare.   However, I’d always had the “entrepreneurial spirit,” and the company was recommended by a more experienced colleague I trusted (who’d also bought in), so I decided to take the plunge and devote a hefty chunk of my bartending (grad school evening job) income to the cause. Almost a decade later, it’s been a tax deduction for me every April, as the company has lost money year after year.  The lowlight came when the vice president walked off with $80,000 to waste on strippers and cocaine, as us unenthused investors were told.  Apparently, when it comes to venture capital, there are “angel investors” and “poor grad students who accidentally fund guys who like boobs.”  It didn’t take me long to figure out which category I was in (although I did take time to consider that $80K is a lot of $1 bills).

I’ve learned lessons from books, DVDs, seminars, training people from all walks of life, and lifting myself – but throwing $5K down the toilet when I really didn’t have it to spare actually ended up teaching me a valuable lesson: no matter who you trust, the only person you can really count on is yourself. This can really be applied to just about any walk of life – from business (obviously) to personal development.  Every decision you make in life is really a balance between trust and complete self-reliance. When you hire an employee, it’s because you trust that he or she will do a good job with clients and customers at the level you expect.  Otherwise, you’d have to extend hours and do everyone yourself…24/7/365. When you go to church and put a few dollar bills in the collection plate, you trust that everyone who touches that money along the way will, in fact, ensure that it goes to the right place.  Otherwise, you’d have to hand deliver your donation each week. When you go to the doctor, you trust that he or she has been educated properly and is thorough enough to give you a diagnosis that might save your life.  Otherwise, you have to get second opinions – or try to diagnose yourself.

Heck, even as you read this newsletter, you trust that I know my arse from my elbow (and in light of my stellar investment story from above, a lot of you are probably second-guessing yourselves already). Catch my drift?  Your life is really a series of dependencies on others, as much as you might hate to admit it. This applies to your strength and conditioning program in a big way. When you go to the gym, you trust that the ownership of that facility has properly maintained that equipment so that it’s not going to break while you’re using it.  Otherwise, you’d be checking out each piece of equipment meticulously between each set. When you connect with a training partner, you trust that he or she is going to be as motivated as you and push you to be better.  Otherwise, you’re lifting by yourself. When you purchase a fitness product, you trust that the author has the experience necessary to create a program that’ll deliver the results you want in a safe and timely manner. How do you ensure that your strength and conditioning program (or any aspect of your life) doesn’t end up as a series of failed dependencies on others? 1. Review the résumé of anyone you’re considering. When it comes to selecting people to work at our facility, the résumé is something that gets you a foot in the door – much like an academic transcript or SAT score might impact college admissions.  At the end of the day, how you act during an interview and perform on the job is more important to me.  For you, though, if you’re looking to purchase a fitness product, check on the background of who created it.  Are they training people – or have they at least done so in the past?  Or, are these hypothetical programs? 2. Look for a track record of success. This might seem synonymous with checking on a résumé, but it’s actually different.  I’ve known people with tremendous on-paper accomplishments who couldn’t cut it in the real world because these achievements didn’t translate to a different realm, or because their previous success had made them complacent and apathetic.  Sadly, I’ve also met people who have forged résumés altogether.  Do your homework by seeking out testimonials and asking around – and that’s where #3 will come into play.

3.  Surround yourself with as many positive – and insightful – people as possible. Your first impression is usually the correct one, but it never hurts to have additional perspectives from those around you.  While there’s no way you can ever guarantee that all the advice you get is good, consistently reevaluating the relationships you keep can be really valuable – not only in terms of making sure that you have the best advice on hand, but also in determining if you need to get someone’s negativity out of your life.  Not every friendship is going to work out, not every business dealing will be a good fit, and not every book/DVD will appeal to you.  The more you can “hone in” your social circle, the better the decisions you’ll make – whether it’s in avoiding the extra slice of chocolate cake, deciding to go for the PR bench press on a day when you could have slacked off, or buying book “X” instead of DVD “Y.” 4. Look for a way out; there should always be a fall-back option. You can test-drive the care before you buy it.  You can find a new training partner if things aren’t working out.  You can always fire an employee if they aren’t the right fit.  Many products have money-back guarantees. 5. Only delegate within your comfort zone. Learning to delegate was the absolute hardest thing for me when we opened Cressey Performance and I had co-owners and employees for the first time in my life.  It took some time, but now I have people doing everything – billing, scheduling, taxes, maintenance, answering the phone – that doesn’t allow me to effectively leverage my strengths: assessments, program design, and coaching.  Comfort in this regard doesn’t magically happen; it’s something that develops over time. To bring this lesson to a close, look back at my botched investment and apply these five principles to it. I didn’t even know the president or vice president of the company, and therefore never checked their résumés (#1).  They’d never run a business before and had no track record of success (#2).  Rather than running my idea by multiple people, I went on the basis of one colleague – who was more of an acquaintance, anyway (#3).  There was no fall-back option, so with this being my first investment opportunity, I would have been smarter to go with something more low-risk, such as investing in stocks/bonds rather than a brand new company (#4). I instantly delegated everything, and to people I didn’t even know!  There was no easing into it (#5). I deserved to lose my money; I was an idiot. 17 Reasons I’m Excited for 2011

With the new year upon us, I got to thinking about how excited I am for all that 2011 has in store for me – and thought that it’d make for a good post to kick off the year.  Here’s why I’m excited: 1. Being Married – My wife, Anna, and I got married on October 3, and it was just the tip of the iceberg in a whirlwind year (new job for her, wedding planning, new house, new puppy).  Both of us are pretty excited for a low-key 2011 where we can just hang out and enjoy one another’s company!  And, we left our honeymoon for this year (I couldn’t escape for that long during the baseball off-season), so we’re excited about that.

2. The Continued Growth of EricCressey.com - I really enjoy writing, and each year, this website grows – which means I get to share my passion and interact with some very cool people.  Here were 2011’s year-end statistics for EricCressey.com: 450,791 unique visitors 1,106,748 visits 2,901,970 pages 2,730,922 hits Thanks to everyone who visited the site this year! 3. The book I’m reading now: The 4-Hour Body. Tim Ferriss has become a good friend, and I was fortunate enough to be one of those who received an advanced copy of The 4-Hour Body prior to publication.  With the crazy goings-on at CP as well as the holidays, I’m just now getting a chance to read through it and give it the time it deserves – and I must say that it’s fantastic.  Tim does an awesome job of providing “info-tainment;” his entertaining writing style will keep you reading, and the background research he put in to this book will guarantee that you walk away with some ideas that will immediately benefit you.

4. The book I’m reading next: The New Rules of Lifting for Abs. As with Tim’s book, I got a copy of The New Rules of Lifting for Abs in advance, but haven’t even had a chance to open it up.  As with any Cosgrove/Schuler collaboration, though, I’m sure it’ll be high quality and a huge hit.  I’m looking forward to checking it out.

5. Cutting Back on Travel – 2010 was a crazy busy year for me personally – from buying a house, to moving, to planning a wedding, to getting married, to getting a puppy.  These “firsts” wouldn’t have been tough to pull off normally, but it seemed like every time my wife and I encountered one of them, I was getting ready to hop on a plane to go do a seminar somewhere.  As such, I’ve started turning down a lot more seminar opportunities not because I don’t enjoy doing them, but simply because the travel wore me out in 2010.  I will, however, still be traveling some – but this year, it’ll be with my wife…and we’ll be traveling for fun! 6. Another Year on the Perform Better Tour – While I may be cutting back on seminar travel, I wouldn’t miss the Perform Better Summits for the world.  I’m still waiting on final confirmation of which cities I’ll get in 2011, but I can say definitively that these are some of the best continuing education opportunities in the fitness business and that I thoroughly enjoy all of them – from the information to the great people I always wind up meeting.  Hopefully, I’ll get to meet some of you in person thanks to Perform Better this year.

7. Continuing on my Postural Restoration Institute Journey – I’ve spoken a bit in the past about the Postural Restoration Institute and how it dramatically impacted the way we evaluate and program for many of our athletes and clients.  In fact, I’d go so far as to say that it has been some of the best continuing education money I’ve ever spent.  I’ve only gone through two of their seven courses, though, and am excited to learn more.  I’ve covered Myokinematic Restoration and Postural Respiration, and already on the agenda for 2011 is Impingement and Instability. If you’re a physical therapist, athletic trainer, or fitness professional and haven’t seen any of their stuff already, I’d highly encourage you to check it out. 8. The New Cressey Performance – I’ll have pictures of the newly-renovated Cressey Performance soon, but suffice it to say that adding 1,000 square-feet can go a very long way.  I’ve finally got my own office at the facility, which I know will make things a lot easier moving forward, but even beyond that, just getting a bit more space can really change the “flow” of the facility to make it more coaching friendly.  We see all sorts of articles and presentations on how to coach, but nobody ever considers how the set-up of your facility can make your coaching duties remarkably easier or more difficult. On top of that, Cressey Performance is busier than ever, with double digit percentage growth again in 2010.  Thanks to everyone for your continued support! 9. Relishing my Fantasy Football Championship – In the most impressive managerial run in Cressey Performance Fantasy Football history, I crushed the competition this year.  This trophy will reside on my desk for the entire year.  Those of you who visit CP can have your picture taken with it, if you’d like.

10. Doing more charity work – I’ve helped out here and there with various charities since I moved to Boston in 2006, but in 2011, I’m excited to do much more – and I’m in a position to do more now, too.  Nowadays, I can use my exposure and expertise a lot more to help – and thanks to my work with Kevin Youkilis, I can work directly with his great charity, Youk’s Hits for Kids. Along those lines, those of you in New England might be interested to check out his February 3 event at the State Room in Boston.  The CP staff will be there along with a bunch of pro athletes, Tony Gentilcore, actors, Tony Gentilcore, musicians, Tony Gentilcore, comedians, and Tony Gentilcore.  For more information, check out YouksKids.org. 11. Click here for more information. 12. New Projects – In 2010, I introduced two products: Optimal Shoulder Performance and Show and Go: High Performance Training to Look, Feel, and Move Better. For me, a product every six months is a pretty good “pace,” as I don’t want to become one of those guys who puts out mediocre stuff every single week.  As of right now, the only confirmed project for 2011 is a collaborative one with Mike Reinold and Mike Robertson.  I am thinking, however, that this is the year that I finally create a baseball-specific product in light of the fact that it’s 80-85% of our clientele and what I do all-day, every day!  Only time will tell! 13. Continued Show & Go Feedback – Speaking of Show and Go, it was released in late September, and since it’s a four-month strength and conditioning program, we’re coming up on the point in time where I start getting loads of emails from those who have wrapped up the program and have results to report.  I get a lot of feedback along the way, but it’s awesome to hear where things end up when the entire program is complete.  So, to those of you doing the program, please pass along your results!

14. More Writing at T-Nation – I only published two articles at T-Nation in 2010, and I don’t plan to repeat that poor output!  I’ve already been contacted by them about doing a monthly piece, and while I’m not sure that my schedule will allow me to get one to them every month, I definitely expect to be blowing that 2010 total out of the water.  I’ve already submitted one and have two more in the works.  I owe a lot to the folks at T-Nation and Biotest for the opportunities and exposure they’ve afforded me and hope to continue to return the favor with good content for years to come. 15. Watching Tank grow up – Our puppy, Tank, is about five months old right now, and he’s awesome.  He is pretty much housebroken, and definitely man’s best friend.  As you can tell, he loves hording his toys.

16. The 2011 MLB Season - In addition to the fact that my team (the Red Sox) is looking good, we have quite a few clients who are on the cusp of big league debuts, so I am excited to get out to see them play in the show and enjoy the fruits of their off-season labor.

17. The 2011 MLB Draft - Let's just say that I very well might just stay home and hit refresh on my computer over and over again during the two days in June that make up the MLB draft.  We have a lot of talent athletes - both high school and college - training at Cressey Performance who will be getting calls.
