Preventing Baseball Injuries: Actions Speak Louder Than Words

We're at a point in time where just about everyone knows that throwing a baseball year-round is a bad idea. Moreover, we know that it's best for kids to avoid early sports specialization. 

Dr. James Andrews has been outspoken against early specialization and year-round throwing for roughly a decade.

John Smoltz devoted a big chunk of his Hall-of-Fame acceptance speech in Cooperstown to discouraging kids and parents from early specialization and year-round baseball.

Seahawks coach Pete Carroll recently referred to the trend of kids playing only one sport as "an absolute crime."

USA Baseball launched their Pitch Smart campaign - featuring an advisory board of many MLB team doctors and athletic trainers - to prevent overuse in youth baseball.

All the way back in 2006, a landmark study by Olsen et al. clearly demonstrated strong associations between injuries requiring surgery and pitching "more months per year, games per year, innings per game, pitches per game, pitches per year, and warm-up pitches before a game" as well as showcase appearances during adolescence. Overuse is the one factor that predicts injury over and over again in the research.

A 2011 study demonstrated that players in warm weather climates had less shoulder strength and more problematic range-of-motion adaptations than those in cold weather climates. And, speaking from personal experience from having Cressey Sports Performance facilities in both states, it's been far more challenging to develop players in Florida than it is in Massachusetts. There is simply too much baseball competing with general athletic development.

These are just a few examples, too. Hundreds of professional athletes have spoken out against early sports specialization. College coaches have in some cases refused to recruit one-sport athletes. And, there are more anti-specialization posts and websites freely available on the Internet than one could possibly imagine. Yet, the problem isn't even close to going away, and injuries still at all-time highs.

Now, I can understand how some players, parents, coaches, and scouts don't stay on top of the American Journal of Sports Medicine and might have missed this important information. What I can't understand is how they'd miss it when the world's most recognized orthopedic surgeon is speaking out against it. Or how they can miss it when one of the most accomplished pitchers of the last century devotes the biggest media spotlight of his life to bashing early sports specialization. Or how they'd overlook one of the premier coaches in the NFL so vehemently putting down the practice. Or how a governing body like MLB would devote time, money, and resources to a problem that they think will have a significant negative impact on the future of the game beyond just the billions of dollars that are already being wasted on players on the disabled list.

The problem is not a lack of knowledge; the problem is a lack of action and consequences.

When you were a little kid and stole a cookie from the cookie jar - even after your mother told you it was off limits - you got punished for doing so. If you didn't have consequences, you'd keep stealing cookies. Unfortunately, this isn't an option with youth baseball. Really, the only consequence is injury, and it's surprisingly not that great a teacher.

A lot of kids and parents continue to make the same mistakes even after an arm surgery and extended layoff. They've been brainwashed to think that the only way kids can succeed in baseball is to play year-round to keep up with other kids and get exposure to college coaches and pro scouts. There are too many coaches, showcase companies, and scouting services lining their pockets by lobbying hard to make these false assumptions stick. 

If knowledge ("eating too many cookies is bad for you") isn't working, and it's hard to deliver consequences, what's the next step? You've got to make it really hard to get to those cookies - and they better taste like crap if you do manage to do so. 

Stepping away from this analogy, the big governing bodies that matter need to step up their game. Here are six quick changes that I personally feel could have a profound impact on reducing injury rates across all levels:

1. Major League Baseball needs to implement a high school scouting "dead period" from October 1 through January 1. It is entirely hypocritical for MLB to push PitchSmart, but turn a blind eye when literally hundreds of scouts are showing up for October-December showcases and tournaments that directly compete with the PitchSmart initiative. Most of the highest-profile players aren't even attending these events anymore (advisors know it's an unnecessary injury risk), and there is absolutely nothing a scout would see in November that they can't see in the spring during the regular season.

2. MLB should also mandate that no pitcher can throw in more than three consecutive games - including "getting hot" (throwing in the bullpen, but not entering the game). Some might criticize me for this, but after extensive interaction with relievers at this level, I firmly believe that bullpen mismanagement is one of the biggest problems in MLB pitching injuries. Fans and the media only see the actual number of appearances, but when you factor in the number of times a pitcher "gets hot" without entering the game, you have relievers who are literally throwing over 120 times in a season.

3. The NCAA needs to implement innings limits on freshman and sophomore pitchers. Keep freshman pitchers to 120 innings and sophomore pitchers to 140 (combining the college season and summer ball). Additionally, any pitcher who throws more than 120 innings during the spring/summer should have a mandatory 60-day period of no throwing prior to starting fall ball.

4. The NCAA should also implement a conservative pitch count limit for college starters. I think 130 is a good place to start, and while I still think it's unnecessarily high, it reins in those coaches who'll leave a guy in for 150+ pitches. Sadly, this happens far too often in college baseball these days, and there are zero repercussions (although I do commend ESPN's Keith Law for always calling these coaches out on Twitter).

5. State athletic associations in warm weather climates need to structure high school seasons to allow for athletes to compete in multiple sports. As an example, in Massachusetts, the high school baseball season begins on the third Monday in March, while the first basketball practice is November 30. If a high school basketball player wants to play baseball, he might only have a 1-2 week overlap during that month - and it only happens if his team goes deep into the playoffs.

Conversely, the high school baseball season here in Florida begins on January 18, while the last regular season basketball game doesn't occur until January 30. The state championship games take place February 23-27 - which is roughly halfway through the baseball season! There is absolutely no reason for a high school baseball season (in which teams play about 30 games) needs to start prior to March 1.

That extra six weeks would make a huge difference in getting more baseball players to also participate in winter sports and help to get a baseball out of young hands a bit longer. And, you'd see a lot more players well prepared on day 1 of baseball tryouts because they'd have more off-season preparation under their belts. It would simply force teams to play three games per week instead of two; this is exactly what's done in Northern states (and they'll sometimes play four, if weather interferes).

6. Similar to point #4, state athletic associations should also have regulations on permissible pitch counts for high school arms. I think 115 pitches is a good number.

Closing Thoughts

I should note that I actually think Little League Baseball does a solid job of disseminating information and including specific regulations within the game and between games. The changes - at least in my eyes - should rest with high school athletic associations, the NCAA, and Major League Baseball. Impact will come from the top down.

As you can see, with only two exceptions, I'm much more about managing the competitive year than I am about micromanaging pitch counts. And, the two pitch count recommendations I put out are remarkably conservative and just reaffirm common sense (which, unfortunately, isn't so common anymore). Pitch counts alone haven't proven to be tremendously effective, but do have a place when implemented alongside guidelines for managing the overall baseball calendar.

There is absolutely no reason for skeletally immature middle and high school baseball players to have longer competitive seasons than professional players.

Understanding Scapular Positioning in the Throwing Motion

At a recent conference, Eric Cressey gave a presentation that tackled the importance of baseball professionals understanding scapular mechanics and the integral role they play in the throwing athlete’s kinetic chain. Eric Schoenberg also recently showed a great drill to incorporate scapular motion into the kinetic chain of activity. Given that I’m the third member of the Elite Baseball Mentorships team with these two, I figured I might as well chime in to highlight its importance from my perspective as well.

This is an important discussion to have because it can help demonstrate the need for all phases of development to work together to keep the high-level thrower operating on all cylinders. If we’re all speaking the same language, we can work to build the athlete’s awareness for their overall movement and integrate the education from the warm-up through the initial phases of the throwing progression.

If we’re all saying different things to the athlete using our own jargon, it’s easy for them to misinterpret the carryover of certain drills, exercises, and concepts across channels. If we all lay down similar verbiage in our conversations with the athlete regarding their prehab work, dynamic warm-up, strength training and throwing motion, it makes it a lot easier for them to appreciate the importance each piece holds in the puzzle.

In order to get started, let’s look at where the scapula is positioned and introduce its fundamental movements so we can begin to appreciate its role in the kinetic chain.

When looking at the scapula’s position and actions, you have to acknowledge the importance of its relationships with the rib cage and the humerus. These relationships are integral in tying the torso and the arm action together in a high-level throw. These interactions between the thoracic region, scapula and humeral head may be the most overlooked or misunderstood components of the delivery – especially for the average coach who has no anatomical background.

The degree of misunderstanding is mainly because the actions are so subtle and can’t be fully appreciated when the athlete throws with their shirt on. This is why its so vital to have a strength/rehab professional in the mix, who can provide a shirtless scapular screen to give us a baseline on where the scapula lies at rest and how it functions in relation to the movement of the arm.
 

Once you can identify how an athlete presents, you can begin to build a more individualized corrective movement progression. This will serve to help the athlete identify and turn on the appropriate movement patterns to keep the humeral head flush with the scapula through its full range of motion. This is essential in the throw, because of the importance of a “clean” arm action to help alleviate some of the stress involved in the high-level motion. For demonstration sake, here’s an example of a HS pitcher, who throws 88-91, with a relatively efficient arm action for his age.

The ability to create elite levels of hand-speed in a durable manner can be won or lost based on how the humeral head functions in conjunction with the scapula. In my mind, this is the crux of the delivery, where you need to be able to tie the “whip-like” arm action into the sequential actions of the torso.

As the thrower engages his landing position, the kinetic forces of the delivery are beginning to flow up through the chain towards the scapula and arm. It’s crucial at this point for the arm to get set up in a sound position to optimize control of the (glenohumeral) joint in an effort to handle the energy that’s about to drive through that portion of the chain towards release. The “optimal” timing of this set-up will be dictated by how the athlete sequences hip and torso rotation, as well as how much laxity they present with, etc. - but for the sake of discussion, we’ll say landing is a crucial checkpoint.

From here, the key actions that we’re going to break out today are upward rotation and protraction. This isn’t to say that they are more important than the other actions, but throughout the season, throwers tend to lose upward rotation from the stress of the throwing motion. With that in mind, let’s identify what it is and how it works with protraction to aid the durability of the high-level delivery.

This concept is something that EC has written and produced videos about countless times over the years, but it continues to be a point that needs to be reiterated time and again. For those who haven’t seen it, this is a great video to consider in this discussion.

From this video, we’ll take it a step further, so you can visualize how this actually plays out in the throwing motion itself.

As you can see, there is a considerable amount of range of motion and control that needs to be in place if you expect to keep the humeral head “centered” from lay-back through the entirety of the deceleration phase. The challenge here is that we can’t always see how the arm action is working with the shoulder blade. One way to combat this is via communicating with your athletes about where they feel their soreness the day after throwing.

Generally speaking, I like to have guys tell me they’re sore near the medial border of the scapula, in the meat of back, where the scapular retractors are eccentrically controlling the scapula as it moves away from the mid-line. If guys are sore near the back, top, or front portion of the shoulder joint itself, then we’re probably getting too much “joint-play” and the humeral head is gliding and translating away from the center of the socket too much during the throw.

If these other patterns of soreness are presenting somewhere along the line, either the rotator cuff wasn’t doing its job, the scapula wasn’t working in sync with the humeral motion, or the thrower’s motion in general is putting them in positions that aren’t utilizing the correct patterns. In this case, let's assume that we did have a “good” post-throwing stress pattern.

Once we’ve identified that we are using scapular upward rotation and protraction to our benefit to control the socket, now we need to work extremely hard to counteract the eccentric damage associated with these actions. This is where the recovery protocol and the warm-up itself are crucial on a daily basis to make sure we’re getting back both the range of motion that we need, as well as activating it correctly before we begin to throw again.

Building Aerobic Capacity with Mobility Circuits (Another Nail in the Coffin of Distance Running for Pitchers)

If you've read EricCressey.com for any length of time, you're surely aware that I'm not a fan of distance running for pitchers. I've published multiple articles (here, here, here, and here) outlining my rationale for the why, but these articles have largely been based on theory, anecdotal experience, and the research of others. Today, I wanted to share with you a bit of data we collected at Cressey Sports Performance - Florida not too long ago.

First, though, I should make a few important notes that "frame" our training recommendations and

1. Athletes absolutely must have a well-developed aerobic system in order to recover both acutely (during the training session or competition/games) and chronically (between training sessions and competitions/games). It's relatively easy to improve if approached correctly, and can yield outstanding benefits on a number of physiological fronts.

2. As long as the intensity is kept low enough during aerobic training initiatives, it won't compromise strength and power development. I wrote about this all the way back in 2003 with Cardio Confusion, but many industry notables like Alex Viada, Joel Jamieson, Mike Robertson, Bill Hartman Eric Oetter, Pat Davidson, and Charlie Weingroff have done a far better job describing the mechanisms of action in the 12 years since that article was published. Speaking generally, most folks put the "safe zone" intensity for aerobic development without strength/power compromise at approximately 60-70% of max heart rate (Zone 2, for the endurance savvy folks out there).

3. It might be a large amplitude movement (great ranges-of-motion achieved), but baseball is a low movement variability sport. Pitchers are the most heavily affected; they do the exact same thing for anywhere from 6-9 months out of the year (or up to 12, if they're making bad decisions by playing 12 months out of the year). Distance running to me does not offer significant enough movement variability to be a useful training option for developing the aerobic system.

4. The absolute best time to develop the aerobic system is early in the off-season. For the professional baseball player, this is Sep-Oct for minor leaguers, and Oct-Nov for major leaguers. This is one more strike against distance running; after a long season of being on their feet in cleats, the last thing players need is a higher-impact aerobic approach.

With these four points in mind, two years ago, I started integrating aerobic work in the form of mobility circuits with our pro guys in the early off-season. The goals were very simple: improve movement quality and build a better aerobic foundation to optimize recovery – but do so without interfering with strength gains, body weight/composition improvements, and the early off-season recharge mode.

The results were awesome to the naked eye – but it wasn’t until this week that I really decided that we ought to quantify it. Lucky for me, one CSP athlete – Chicago White Sox pitching prospect Jake Johansen – was up for the challenge and rocked a heart rate monitor for his entire mobility circuit. A big thanks goes out to Jake for helping me with this. Now, let’s get to the actual numbers and program.

Jake is 24 years old, and his resting heart rate upon rising was 56 beats per minute (bpm). If we use the Karvonen Formula for maximum HR (takes into account age and resting HR) and apply our 60-70% for zone 2, we want him living in the 140-154bpm range for the duration of his session. As you can see from the chart below – which features HR readings at the end of every set during his session – he pretty much hovered in this zone the entire time. The only time he was a bit above it was during an “extended” warm-up where I added in some low-level plyo drills just to avoid completely detraining sprint work (he’d already had a few weeks off from baseball before starting up his off-season).

When all was said and done, Jake averaged 145bpm for the 38 minutes between the end of his warm-up and the completion of the session.

He bumped up a little bit high in a few spots, but that’s easily remedied by adding in a slightly longer break between sets – or even just rearranging the pairings.

To that last point, I should also note that this approach only works if an athlete is cognizant of not taking too long between sets. If he chats with his buddies and heart rate dips too much between "bouts," you're basically doing a lame interval session instead of something truly continuous. Jake did 44 sets of low-intensity work in 38 minutes. You can't get that much work in if you're taking time to tell a training partner about the cute thing your puppy did, or pondering your fantasy football roster.

Think about the implications of this....

What do you think this kind of approach could do for the foundation of movement quality for a typical high school, college, or professional pitching staff?

Don't you think it might make them more athletic, and even more capable of making mechanical changes easier?

Don't you think they'd be less injury-resistant performing an individualized mobility circuit instead of one-size-fits-all distance running?

Do you think that maybe, just maybe, they'd feel better after an 11-hour bus ride?

Don't you think they'd bounce back more quickly between outings?

Designing a low-intensity mobility circuit like this is not difficult. I have a ton of examples on my YouTube page and in products like Assess and Correct and The High Performance Handbook. Stuff like this works great:

What is difficult for some coaches, though, is admitting that distance running to "build up your legs" is like changing the tires on a car with no engine, or studying for the wrong test. Just because "that's how it's always been done" doesn't mean that's how it has to stay.

Give some of these a try in the early off-season - and even during the season in place of "flush runs." They'll be a big hit with your athletes both in terms of performance and health.

What is a “Big League Body?”

You might be surprised to know that I'm an "outsider" to baseball. I played up through 8th grade, but was actually a much better tennis player. Since they were both spring sports, the decision to go the tennis route was effectively made for me even though I loved baseball as both a player and fan.

Years later, in my first three years of strength and conditioning during graduate school at the University of Connecticut, I actually spent most of my time working with basketball and soccer players. I still loved baseball, but hadn't really been exposed to it "from the inside."

It just so happened that when I went to the private sector after graduate school, some of my first clients were high school baseball players. They had some good results and my phone started ringing off the hook. Now, almost ten years later, Cressey Sports Performance has facilities in both FL and MA, and we work with players from all 30 Major League Baseball organizations.

I'd argue that if I had been an "insider" from the get-go, we never would have been able to differentiate ourselves so quickly. Why? As an outsider, I had to do a lot of listening and observing. I had to ask a lot of questions. And, I was fortunate to NOT be married to personal biases of what baseball training programs should look like.

Distance running for pitchers didn't make sense to me. It was absurd to hear that players shouldn't lift because it'd make them "bulky and inflexible." The list goes on and on - but the point remains the same: good baseball strength and conditioning mandates logical thinking, not just reliance on tradition and personal biases. 

This doesn't just apply to baseball insiders' perspectives on what players should look like, though. Rather, it also must apply to how strength and conditioning coaches - baseball outsiders like me - view what players should look like. And, my decade in baseball strength and conditioning has taught me that you have to emotionally separate yourself from what an athlete "should" look like, especially when dealing with pitchers.

While you might think that every athlete - regardless of sport - needs to be 7% body fat with a 500+ pound squat and 18-inch biceps, you have to throw that perception out the window when you're dealing with baseball players (and many other athletes, for that matter). The truth is that big leaguers come in all shapes and sizes.

I've seen pitchers with 16-inch vertical jumps who throw 95mph. I've seen guys at 25% body fat win Gold Gloves and hit over .300 in the big leagues. I'm not saying that you should just "allow" your athletes to be unathletic, but rather that you need to recognize the following:

Players are often successful because of traits and not just athleticism. 

Maybe a hitter has tremendous sports vision. Maybe a pitcher was blessed with freaky congenital laxity (joint hypermobility) to contort his body into crazy positions to create better deception and get on top of hitters faster. Maybe a pitcher has really long fingers that enable him to throw a great change-up or splitter.

The point is that natural selection definitely plays a big role in success, and it's your job to make sure your training doesn't interfere. Bartolo Colon has pitched in parts of 21 years in the big leagues and won 213 games, and he's making $12.5 million this year. He throws more than 85% fastballs and walk less than one batter per nine innings.

He is also clinically obese and probably has about 50-60 pounds of body fat to lose. Conventional wisdom says that shedding that excess body weight would make him a better athlete, but as I wrote with respect to CC Sabathia, that's a very slippery slope. That extra body weight may help him with absolute power development, and he may also be so accustomed to that larger frame after all these years that his mechanics might negatively impacted if you took a lot of weight off him, especially in a short amount of time. I've actually seen this quite a bit over the years in athletes who have come our way to sort out their struggles: try to get a guy too lean, and you'll deliver a six-pack - and a poop arm to go with it. For Colon, fastball command (and velocity) is clearly more of a priority than flab (or lack thereof).

I'm not telling you that you should let your guys get fat. I'm not telling you that you shouldn't train them for strength. I'm not telling you that we should just assume that the freaks will make it and hard work won't take a non-freak to the big leagues. I'm just telling you that you need to take a step back and consider exactly what makes an athlete a) successful and b) durable.

Guys aren't getting hurt because they only deadlift 450 and not 500 pounds. And, they aren't stuck in Triple A instead of living the big league dream because they're 10% body fat and not 9%. There are such things as strong enough, lean enough, and flexible enough. If they don't meet the minimum standards and they aren't at the level at which they hope to compete, you need to improve these qualities. If they don't meet these minimum standards, but they're already competing at the highest level, then you have to be very careful about how you tinker with things. Subtle changes are the name of the game, and extremes should be avoided. And, you should look for easy gains first.

Improving a pitcher's cuff strength is a lot quicker and easier than adding 100 pounds to his deadlift. Adding 10 degrees of hip internal rotation or thoracic rotation can make a hitter feel far more confident at the plate. Getting 10 minutes of soft tissue work on a gritty shoulder or elbow can be absolutely game-changing for a pitcher who has thrown through chronic pain.

These aren't the sexy training exercises or boast-worthy gains that make for YouTube videos that go viral, but they're the ones that can take athletes to the next level - or keep them at the highest level. I'll take a good serratus anterior over a good six-pack, even if it makes for lame Instagram content. I'll take great end-range rotator cuff strength over big arms, even if it won't make the ladies go wild.

Our athletes are still going to lift heavy weights, sprint, throw med balls, do plyos and agility drills - but it's all part of a larger plan where we appreciate them as individuals with unique needs. If you try to fit baseball players into a physical mould that you've built in your mind, you'll waste training time as you study for the wrong test. And, more importantly, you'll miss out on key performance benefits and opportunities to keep them healthy.

Changing Baseball Culture: A Call to Action

Baseball is a game of ritual and tradition: lucky socks, pre-game meals, stepping over lines, special handshakes, and on-deck habits are all part of the “rhythm” of the game. Unfortunately, other “old-school” traditions are still the norm when it comes to the management and prevention of injury on the diamond. It is clear that we are moving in the right direction with new technologies and smarter training; however, injuries continue to pile up. A difficult question to answer is: are any of these injuries avoidable or are players already “damaged goods” by the time they get to the professional ranks? Some things are out of our control, but clearly we can do better.

Here are four opportunities for us to make a difference:

1. Identify the signs before there are symptoms.

The best form of treatment is prevention. The best rehab for a pitcher is one that does not exist at all. To support this point, a sign is a warning that something bad is about to happen. Some examples of objective signs in an at-risk pitcher are a decrease in velocity, loss of location/command, and ROM changes. This might be a loss of total glenohumeral ROM, internal rotation, or shoulder flexion; scapular upward rotation; elbow extension; forearm supination; and hip rotation. Or, it may be a significant increase in shoulder external rotation (see here and here for details). Some examples of subjective signs are poor body language, lack of confidence, altered communication, and working slower on the mound – just to name a few. Once a pitcher does become symptomatic, we need to take it seriously. I am not implying that we need to baby our athletes (there is enough of that going on!), but on the flip side, the solution is not to ignore the pain and “pitch through it.” As always, the truth is often found somewhere in the middle. In 15 years, I have yet to come across a pitcher that ended up needing surgery that did not first have signs and symptoms that were either missed or ignored.

2. There is no such thing as “normal soreness.”

To piggyback on point #1, the expectation is not that a pitcher will be pain-free 100% of the time. That is unrealistic. There is unavoidable stress and tissue breakdown associated with pitching. If there isn’t, my guess is the pitcher is not throwing very hard! However, I would like to make this point loud and clear: There is no such thing as “normal soreness.” By definition, if things were normal, then there would not be soreness (and certainly not pain). To this point, one could argue that throwing a baseball 100x at 85-100MPH is not “normal,” either, so what can we do about it? Let’s follow this rule: if a pitcher presents with pain, tightness, or fatigue in the front of his shoulder or the inside/outside of their elbow or forearm following an outing, then he needs an evaluation and treatment. If a pitcher presents with soreness in their glutes, core, and posterior cuff, then he needs some rest and a pat on the back for a job well done. Remember, just because the pain or soreness is common doesn’t make it right.

3. One size never fits all.

I can’t think of a situation in baseball (or life) where one approach works for everyone. For example, not every baseball player needs to stretch or “loosen up.” Most players are already too loose or lax and need to gain stiffness and stability with their pre-game routine. They need to warm-up and activate. Yet, at every level, we see teams line up and stretch before games. This robs their bodies of the good stiffness that they have worked to develop in the gym and during the off-season. We need to warm the tissues up and take the body through the appropriate ranges of motion to prepare to play; however, we don’t need to stretch these tissues right before asking them to generate massive amounts of force. For these loose-jointed individuals, throwing, sprinting, and hitting will provide all of the “stretching” that’s needed.

Another example that needs to be looked at is too many reps of strengthening or band exercises done right before activity on the field. If we don’t want to overstretch an athlete prior to playing, then we certainly don’t want to fatigue them. A fatigued pitcher has a 36x higher chance of getting injured than a non-fatigued pitcher. Let’s save the fatigue for the innings on the field and not with hundreds of band reps or a 50-pitch bullpen session before the 1st pitch is even thrown.

4. Avoid this question.

The worst question you can ever ask a pitcher on a mound visit or in the dugout is “How are you feeling?” This same question is asked every day on fields across the world and yields no valuable information. Any pitcher, at any level, will answer, “I’m good, coach.” If they don’t, they are playing the wrong sport.

Instead of asking this question, we should be using our experience as professionals to make unemotional decisions to best help our players stay healthy for the entire season.

It is our job is to acquire as much information as we can through experience and observation to make the best decision possible with the data that we have at that moment. Players lie. It is a way of showing their competitive spirit to stay in games and try to help their teams win. It’s called adrenaline. It’s not their fault. It is our fault for asking bad questions that have no good answers. The pitcher’s job is to get outs, not to decide what soreness is “normal.” That is what what we get paid to do.

Let’s close by comparing injury management in baseball to one of the world’s most successful companies. Apple talks about avoiding the “sameness trap.” This is the thought that if you ask a consumer what they want, they will tell you to do what other popular companies are doing. Steve Jobs worked to avoid this by not asking his customers what they wanted, but instead, giving them what they didn’t know they needed. So, let’s stop asking the same questions and getting the same generic answers and worked towards continuing to change the culture in baseball and help our athletes get better results.

Common Arm Care Mistakes – Installment 6

It's that time of the year when our baseball players are in-season, so things get a bit quieter around Cressey Sports Performance. Sometimes, it's even so quiet that my staff members film videos like this:

As impressive as this reverse stunner was, I actually get even more excited about taking a step back to work "on" the business instead of "in" the business when quiet season rolls around. Often, this work on the business consists of "audits" on everything from assessment, to programming, to coaching cues; we want to know how we can get better. One topic that came up during one of these discussions was the recent trend of fitness professionals and some physical therapists insisting that upper body carrying variations with appropriate joint positioning would suffice for arm care. Examples would include things like Turkish Get-ups, or bottoms-up carrying variations:

While I absolutely love all these exercises, I firmly believe that they are only a few pieces of a larger puzzle - and this brings me to this arm care mistake:

Not selecting exercises that appreciate the true functional demands placed on the shoulder and elbow during throwing.

The problem with the "carries are enough" mindset for shoulder health is that this opinion is heavily predicated on the assumption that we're talking about general population folks who don't have to stabilize extreme positions like end-range external rotation during the late-cocking phase:

A 90/90 External Rotation Hold would be a much more appropriate training strategy that would appreciate the unique joint position demands of throwing, as Eric Schoenberg demonstrates:

Another example would be the crazy distraction forces that occur at the shoulder during ball release:

A rhythmic stabilization at the ball release position probably yields better carryover to the act of throwing:

Most of the research on isometric training shows a 10-15 degree carryover in strength from the joint angle trained. In other words, if you don't train anywhere near end-range external rotation, don't expect to be strong in that incredibly crucial position.

I've only spoken to joint position specificity thus far, though, and there is more to this discussion. Baseball players also need to handle some pretty crazy velocities of arm speed - particularly with respect to shoulder internal rotation and horizontal adduction, as well as elbow extension. Good programs start out by building strength through these patterns:

Once a solid strength foundation is in place, we need to begin to challenge athletes on the velocity end of the spectrum:

Very simply, to keep throwers healthy, you need to challenge both cuff strength and cuff timing - and do so at functional significant positions. In my opinion, just relying on carrying variations doesn't really accomplish either of these challenges correctly, and you can't carry in the positions that really matter.

As a final point, I'll add that I think it's a leap of faith to say that a largely reflexive muscle group (the rotator cuff) will automatically fire across an entire population when we know that structural deviations from normalcy (e.g., asymptomatic cuff tears, labral pathology) are widely prevalent.

Carrying and supporting variations are absolutely fantastic and I'll continue to use them a ton, but in my opinion, it's shortsighted to say that they can serve as a complete replacement to more "functional" arm care drills that replicate the forces and positions our players encounter on the field.

Projecting the Development of High School Pitchers: Training Habits Matter

It happens every year. Inevitably, I talk to college coaches about players with whom I work, and without fail, the conversation always comes back to the question: "what type of kid is he and how hard does he work?"

These are two loaded questions and they’re becoming incredibly important in the evaluation process for college coaches. Because the recruiting timeline is getting faster paced every year, coaches are dipping into increasingly younger talent pools to get commitments. This process is forcing coaches to become more reliant on their ability to project what a 15 or 16 year old pitcher is going to look like three years down the road and project what that player might become at ages 18-22 in a new environment. If this is the case, then it becomes essential for coaches to be able to balance who the teenage boy is that he is currently watching, with the man he’s inherently going to become in a few years under his watch.

In order to do this, you need to have the ability to look at the individual’s actions and behaviors, as movement patterns that you think indicate potential for continued growth as this player moves forward. This topic could expand into a entire book, but I’m going to simplify this thought and condense the discussion down to one athlete to help demonstrate the point I’m trying to make.

In this instance, I want to highlight an athlete I've coached over the last few years and show what a drastic difference a year can do in the context of mechanical development. I think it will bring to the forefront how important it is to allow a player to grow into himself and not force the process for these athletes. While doing that, I want to flush out some of the character traits that are involved in refining this process on a larger scale.

Here’s a video of the same athlete one year apart (we’ll break it down in detail later in the article):


To give you some context, you have a 5’9 150lb sophomore on the right and a 5’10” 170lb junior on the left. The 150lb sophomore version of this pitcher pitched around 78-82mph with an above-average change-up and above-average command. This allowed him to develop into a consistent high-level performer on the 16U summer circuit playing in national travel tournaments, but yet the phone isn’t ringing off the hook for this type of 16U player unless he shows “projection” in the body or above average velocity now (neither of which apply to him).

I can understand how it would be very easy to write this type of player off as "average," because every high school RHP in America throws 78-82mph. As such, how could you possibly see this player and offer him a scholarship to play in college? Well, if you’re paying attention, and look at this pitcher one year later with an additional 20lbs on his frame and see that the delivery has continued to refine itself, you’re going to begin to gather a positive sense of direction for this athlete and realize that this RHP is going to conservatively throw 84-87mph this year with a very good chance to throw harder.

Now, 84-87mph still may not get a lot of people excited in this day and age, but I would go out on a limb and say that by the time this athlete is physically maturing in college, you’ll be looking at an 88-90mph RHP with three pitches, who knows how to compete in the strike zone at a high level because he wasn’t blessed with velocity from an early age. There’s a spot for that type of pitcher on any college staff; I don’t care who you are.

One could also certainly say that’s a large leap to make in projecting a 5’10” 170lb pitcher, but it all comes back to knowing what type of person they are and how hard they work. That’s why I think intimate knowledge of their overall training activity is crucial, because you can find out if this player is willing to go away from the “fun” part of developing their skills and identify that they’re willing to buy into a much larger process to make themselves a more technically proficient player on the field.

This is important, in my eyes, because there are only so many reps you can expect a thrower to execute, due to the stressful nature of the activity. So, in order to maximize the efficiency of their development, they have to be able to handle concepts that transcend the actual throwing process itself to be able to refine their throwing motion. If they can grasp why learning how to create stability is important, or why learning to manage their tissue quality on a daily basis will increase their training capacity, then you can give them larger and larger windows to create adaptation as an athlete on the field.

Take the athlete in the video, for example. He’s becoming one of the most consistent performers on the field, and it’s no surprise, because he’s learning to become one of the most consistent athletes in the weight room as well. If you are familiar with the pitching delivery, you’ll notice that he has upgraded at least four critical components of the throwing motion:

  • Postural control of his leg lift/gather phase
  • Rhythm/timing of his hands and legs working together during his descent into the stride phase
  • Lead leg stability and postural control from landing to release
  • Ability to maintain integrity and directional control of his deceleration phase

The interesting piece of these four components is that three of these are reliant on the athlete improving his overall ability to create stability in the delivery. At Cressey Sports Performance, I talk with our athletes all the time about understanding if their adjustments are mobility, stability or awareness issues. In this instance, we probably had both stability and awareness issues to resolve. The thing is, once you’re aware of the issues, it still takes deliberate work to iron out a stability problem in the delivery, which is why the athlete’s training habits are so important. Simply throwing the baseball over and over again may help you with your timing and repeatability, but we need to actively attack the strength training if we expect to impact an athlete’s pattern of stability in the throw.

In order to examine this a bit further, let’s walk through each of these components and identify a couple key things in video form:

Postural Control during Leg Lift/Gather Phase

Rhythm of Descent into Stride Phase

Stability from Landing to Release

Control of Deceleration

Now, don’t get me wrong: there’s obviously a long way to go for this athlete to get to 90mph. However, when you look at the development of this individual in the last 365 days, and you consider that there are over 730 more days before this athlete will even play his first college baseball game as a freshman, it becomes that much more important to know who the athlete is. Will the player you’re recruiting be comfortable with who they are, and become stagnant in their development, or will he use his time efficiently to keep improving both on and off the baseball field?

In the short time that I’ve been doing this, I’ve found that there’s usually a progression for athletes that involves learning how strength training can benefit them. It usually starts with showing up to the weight room from time to time thinking that’s good enough. Once they start plateauing there, they realize they actually need to be consistent in showing up to the weight room to make gains. The problem is, they eventually start plateauing there as well, and if they decide they really want to be good, they proceed to make the all-important psychological jump, and realize it’s not good enough to just show up to the training environment anymore. They realize they need to make positive decisions in their daily routines in order to make the most of every training session, whether it’s on the field or in the weight room. If they’re not willing to do that, there’s always someone else who is, and it doesn’t take long before these athletes are passing them by and they’re left wondering what happened?

When the athlete makes the jump from simply showing up to giving a consistent effort to make positive decisions for themselves inside and outside of the training environment, it becomes real easy to tell a college coach, "This is a guy you want, not only on the field or in the weight room, but in your locker room as well."

Timing Adjustments and Their Impact on the Pitching Delivery: A Case Study

Today's guest post comes from Matt Blake, the pitching coordinator at Cressey Sports Performance in Massachusetts. Matt is an integral part of the Elite Baseball Mentorships team.

I recently Tweeted out a picture of some mechanical changes a pitcher had made and it received a lot of responses. As such, I decided I would follow up with a little more depth and context to this particular picture to help shed some light on the thought process that goes into making mechanical adjustments. So, for starters, here’s the picture in question, with the left side being the original delivery and the right side being the revised version.

Typically, when discussing pitching mechanics, I avoid using still shots, because they can be very misleading. In this particular case, there were some substantial changes that were made in this landing position, which I thought encapsulated a lot about the enhanced movement quality of the delivery as a whole, which we’ll unpack in further detail here.

For those familiar with the pitching delivery, the first thing that should jump out at you is the extremely late arm action in the initial delivery. This could be classified as an “inverted arm action” at landing, where in this case, the elbow isn’t necessarily hyper-abducted (elevated) above the shoulder, but the hand is definitely below the elbow. In a Cliff's Notes version, this positioning is generally regarded as increasing stress on the shoulder and elbow. This is in part due to the orientation of the humeral head in the socket at landing, as it’s in a position of excessive internal rotation and pinned into the front of the socket. As a result, we’re not in an optimal position to get the rotator cuff to function to center the head for a clean ball in socket rotation.

This is coupled with the fact that we’re adding more torque to the joint since we have more range of motion involved in getting the hand to full lay-back before accelerating to release. That being said, there are plenty of pitchers who throw very hard and have successful big league careers pitching with an inverted pattern, and the reason they throw so hard may very well be due to their inverted pattern, so you have to constantly weigh the risk/reward of making mechanical adjustments for pitchers.

As an example, Billy Wagner had an inverted pattern and multiple injuries, but was hitting 100mph before it was industry standard to hit 100mph - and he accumulated 422 saves in a successful big league career.

When weighing this potential risk/reward, some of the questions might include:

  • Where is this pitcher currently in the developmental process?
  • What type of stress does he currently report during or after throwing?
  • What can we gain by making adjustments?
  • What do we have to lose by adjusting this current delivery?

These are important questions to consider, because you’re obviously not going to take a big leaguer at the tail end of his career, and adjust what has got him to that point. Conversely, you might adjust a 15yr old high school pitcher, who throws hard, but has erratic command and reports a high level of stress after he’s done throwing.

In this particular case, we had a sophomore in college, who had a track record of success in high school, and was looking to establish his role in a very competitive program with a strong history of winning. His contributions as a freshman were limited in part due to command issues and his velocity would be erratic going anywhere from 82-90mph on any given day.

With these considerations in mind, it became apparent in looking at the the delivery in its current state, that these mechanics might be a limiting factor in commanding the ball at a competitive level, as well as sustaining his velocity on a consistent basis. On the flip side, though, if we reduce the inversion in his arm action, we may lose a mph or two of velocity initially, as we learn to “re-tension” the delivery and create force in a different manner. In order to fully comprehend these issues, let’s take a look at this delivery in full:

As I stated in the video, the crazy thing about this delivery is that for how extremely late that arm action looks in that still shot, it’s really a misrepresentation for how much I like the feel of this delivery as a whole. There’s a lot of quality movement that’s “loose” in nature, and this athlete has a good feel for creating “extension” in the throw, so we really don’t have to adjust the integrity of his movements, but more the timing associated with some of the actions, and at the crux of it, the athlete’s mindset for creating leverage in his throw.

If you look at where this delivery starts to break down, it’s in the excessive “counter-rotation” of his shoulders that creates too much length in the throwing arm and that couples with an exaggerated extension of the back leg into landing.

As a result, the hand can’t catch up and “get on top of the ball” at landing and our pressure into the ground ends up being poor. This combines to create an issue for the stabilization pattern as a whole now, because the front leg can’t brace to create a fixed point of rotation to anchor the throw, as it has to allow for the torso to translate forward in an effort to create time for the hand to get into position behind the ball. So, as you can see, by the front knee ending up working into a more flexed position, we’re diffusing the ground force reaction we’re trying to convert into rotational power, and the pelvis loses its leverage on that front hip, flattening out our rotation. When this happens, you’ll notice that the path of the hand is actually diverted wide instead of keeping an efficient driveline through the target. Without a firm landing position that allows us to accept force properly, and keep the rhythm of our sequencing intact, our command and velocity will continue to be erratic in nature.

Once we identified these issues, we had to rule out that there wasn’t a mobility or stability issue that was limiting our ability to move through more functional positions. In this particular case, mobility definitely wasn’t the issue, and even though the stabilization pattern was currently poor, the athlete did have the ability to stabilize. It really just came down to his awareness for what he was trying to accomplish. So, once we came to agreement that these were things that could be fixed and would be beneficial to his development in the long run, we had to start re-organizing the focus of his repetitions.

Anytime you’re making changes, it’s essential to understand root causes and not just symptoms. For me, the inverted arm action was a symptom of a misdirected focus in the delivery. We needed to make the focus less on length and extension in the throw and more on strength in the landing and properly sequencing his rotations through the chain. By creating a stronger stride pattern and tying the timing of the arm path into the lower half sequencing, we would have a more connected and repeatable delivery that had a more efficient stabilization pattern. Let’s take a look at what shook out over the next seven weeks and then we’ll discuss some of the altered components.

As discussed in the video, the first thing that should stand out in the revised delivery is the compactness of the arm action, and from there, the angle of the ball flight out of his hand. And, to be honest, I could run through every drill that we did to get him to this point, but I don’t know if it’s really the drills themselves that are important. I think we could have accomplished this in a multitude of ways, as long as we kept the focus on cueing him to be “strong into the floor.”

Now, that being said, we definitely used versions of the “stride drill” to coordinate the rhythm of the back-hip rotation and arm action, and we did our share of step-behind shuffles to speed up his timing and learn to accept force properly upon landing, but if the focus on trying to create force into the ground and working from “top-to-bottom” on the baseball wasn’t in place, I don’t think either of those drills would have mattered.

Changing his focus and “pre-throw vision” for what his ball flight should look like helped him organize his body into this revised delivery. By placing the importance on being “strong into the floor”, it didn’t allow him to put himself into these overly extended positions, whether it be the lower half or the arm action, as he came to understand these weren’t “strong” positions. Ultimately, understanding the importance of landing in a position that allowed him to accept the force and transfer it up the chain was crucial in this process.

At the end of the day, the most important part of making any type of delivery change is getting “buy-in” from the athlete himself. It doesn’t matter what I think a delivery should look like unless the athlete understands and accepts why it’s important for him to make these changes, because ultimately he’s the one who has to throw the baseball.

In this particular case, we had a college pitcher who is on the cusp of turning himself into an impact pitcher in a competitive college program. If getting himself into more efficient positions in his delivery allows him to command the baseball more consistently, and he can reduce the erratic nature of his velocity, he’ll give himself a real chance to be a reliable college performer and we can begin to entertain the possibility of becoming a pro prospect.

All in all, I’m really proud of the work this athlete put in over the summer and I think these rapid changes speak volumes about the level of commitment he has to his development, as changes of this magnitude aren’t common in this time frame and they certainly don’t happen by accident. Needless to say, there’s still a lot of work to be done to “own” this remodeled delivery. It needs to become second nature and highly repeatable in order for this athlete to be able shift into a narrow-minded focus on just competing in the strike zone, but I’m certainly excited to see where his continued effort leads him.
